Tekrarlayan gebelik kaybı, tüp bebek başarısızlığı veya rahim içi patolojilerde kronik endometrit olasılığı değerlendirilir.

  • CD138 ve CD56 immünhistokimyasal boyalarla tanı konur.
  • Gerekli hastalarda antibiyotik tedavisi ile birlikte rahim içi irrigasyon (yıkama) ve endometriumu iyileştirmeye yönelik lokal tedaviler uygulanır.

Bu yaklaşım özellikle implantasyon başarısızlığı yaşayan hastalarda oldukça önemlidir.

Tekrarlayan gebelik kayıplarında nedenleri çok yönlü değerlendiriyorum:

  • Genetik testler: Karyotip, WES, WGS
  • Endokrin, tiroid ve immünolojik analizler
  • Histeroskopi ve uterin anatomi değerlendirmesi
  • Plasental histopatolojik analiz (gerekirse ABD iş birlikleri ile)

Amaç, bir sonraki gebeliği en sağlıklı temelde planlamaktır.

Fetal hastalık şüphesi, sıvı birikimi (hidrops) veya çoğul gebelik komplikasyonlarında kapsamlı fetal tıp yaklaşımı sunuyorum.

  • İnvazif tanı: Amniyosentez, CVS, kordosentez
  • Girişimsel fetal tedaviler:

  • Obstrüktif üropatilerde fetal mesane girişimleri
  • Selektif fetosid / fetal redüksiyon
  • Monokoryonik ikizlerde TTTS, TRAP, selektif IUGR takibi

Tüm süreçler multidisipliner, etik ve bilimsel çerçevede yürütülür.

Kadın Hastalıkları ve Doğum uzmanıyım. Özellikle yüksek riskli gebelikler (perinatoloji) alanında üst ihtisas sahibiyim. Klinik pratiğimde:

  • Rahim ağzı yetmezliği,
  • Tekrarlayan başarısız serklaj girişimleri,
  • Abdominal serklaj,
  • Erken doğum riski ve erken membran rüptürü (PPROM),
  • Preeklampsi,
  • Rahim içi gelişme geriliği (IUGR),
  • Gebelikte sistemik hastalıklar (otoimmün, kardiyak, endokrin vb.),
  • Çoğul gebelikler (ikiz ve üçüz gebeliklerde TTTS, selektif IUGR vb.)

gibi tabloların yönetimi özel uzmanlık alanımdır.
